Kennedy Jr. said Monday that the country\u2019s largest health insurers have promised to take steps to streamline the often-criticized&nbsp;<a href=\"https:\/\/www.nbcnews.com\/health\/health-care\/prior-authorization-insurance-denials-patients-treatment-rcna212068\">prior authorization<\/a>&nbsp;process, which can delay or deny patients\u2019 access to care.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Prior authorization is&nbsp;<a href=\"https:\/\/www.nbcnews.com\/health\/health-news\/doctors-patients-try-shame-insurers-online-reverse-previous-authorizat-rcna101073\">a cost-cutting tool<\/a>&nbsp;used by health insurers that requires them to sign off on tests, procedures or drugs before patients can get them.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">The insurance tactic drew renewed attention last year after the&nbsp;<a href=\"https:\/\/www.nbcnews.com\/business\/business-news\/unitedhealthcare-sued-shareholders-reaction-ceos-killing-rcna205550\">fatal shooting of Brian Thompson<\/a>, the CEO of UnitedHealth\u2019s insurance arm, in New York City.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Patients and doctors say prior authorization creates too many roadblocks, forcing people to wait days or weeks for needed treatments or&nbsp;<a href=\"https:\/\/www.nbcnews.com\/news\/investigations\/-lived-health-insurance-companies-deny-cancer-care-patients-rcna182611\">denying them altogether<\/a>.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">About 1 in 6 insured adults say they\u2019ve had prior authorization problems,&nbsp;<a href=\"https:\/\/www.kff.org\/affordable-care-act\/issue-brief\/consumer-problems-with-prior-authorization-evidence-from-kff-survey\/\">according to a survey from KFF, a health policy research group<\/a>.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Kennedy said a number of large insurance companies \u2014 including Blue Cross Blue Shield Association, Cigna, Elevance Health, GuideWell, Humana, Kaiser Permanente and UnitedHealthcare \u2014 pledged to make the changes, which will be implemented across private insurance, Medicare Advantage and Medicaid.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">AHIP, a health insurance industry trade group, said the changes could benefit 257 million people in the United States.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Experts stressed, however, that prior authorization isn&#8217;t going away.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">It\u2019s \u201cgoing to streamline it in some incremental ways only,\u201d said Dr. Adam Gaffney, a critical care physician and assistant professor at Harvard Medical School.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Health insurance companies have made similar commitments to fix prior authorization in the past, a fact Kennedy and Dr. Mehmet Oz, administrator of the Centers for Medicare &amp; Medicaid Services (CMS), acknowledged at a media event Monday. In 2018 and again in 2023, health insurance companies made commitments to reform prior authorization, Oz said, but many failed to implement such reforms.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Asked what\u2019s different this time, he said, \u201cThere\u2019s violence in the streets over these issues,\u201d alluding to the shooting of Thompson.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">AHIP \u2014 formerly America\u2019s Health Insurance Plans \u2014 detailed the steps health insurance companies committed to in a&nbsp;<a href=\"https:\/\/www.ahip.org\/news\/press-releases\/health-plans-take-action-to-simplify-prior-authorization\">news release<\/a>&nbsp;earlier Monday.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Starting next year, if patients switch insurance plans while they are receiving treatment, their new plans will honor the existing prior authorizations for similar care for up to 90 days. In addition, next year, insurers will need to provide easier-to-understand explanations when they deny authorizations and offer guidance about how to appeal.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Insurers have pledged to have medical professionals review all authorization denials, though AHIP said that is something that insurers say they already do.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Insurers have also committed to streamline the prior authorization process by making it easier to submit requests online by 2027. At least 80% of electronic requests will be answered in real time by 2027, AHIP said.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Plans may also reduce the number of medical services subject to prior authorizations in certain cities or states \u2014 though specific commitments about what that might include were lacking.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Chris Klomp, who leads the Medicare program at CMS, said the agency would like insurers to roll back prior authorization requirements for common services, including colonoscopies, cataract surgery and childbirth.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Kennedy also said CMS is working with insurers to make it easier to get prior authorization for diagnostic imaging, physical therapy and outpatient surgery.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Gaffney criticized the pledges, saying insurers have had decades to change their practices.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">\u201cTalk is cheap,\u201d Gaffney said. \u201cMore fundamental reform will be needed to address the ubiquitous barriers to care imposed by insurance firms.\u201d<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\">Kaye Pestaina, director of the program on patient and consumer protection at KFF, said some of commitments could have direct impacts on patients, such as honoring existing prior authorizations for three months after patients switch insurers.<\/p>\n\n\n\n<p class=\"has-medium-font-size wp-block-paragraph\"><a href=\"https:\/\/www.nbcnews.com\/health\/health-news\/kennedy-says-health-insurers-promise-change-prior-authorization-proces-rcna214515\">Nbcnews<\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>WASHINGTON \u2014 Health and Human Services Secretary Robert F.
